This form will "Hold" your spot for 72 hours. A deposit of $50 is required to "Book" your spot on a Trophy Hunt weekend. Once you complete the registration form below you will be redirected to a "Confirmation Page" that will give you payment instructions (online payments and payments via mail) AND a Liability Waiver Form that must be completed. You will receive additional information via email once you are registered.

WALLEYE 101 TROPHY HUNT TERMS AND CONDITIONS

ALL Participants are required to agree to all Terms & Conditions prior to participation in a Walleye 101 Trophy Hunt.

Please check the new prices for 2012. The new prices reflect the rise in some of our fixed costs, but will also include additional seminar and interaction time on Thursdays, DVD with pictures and video show by the Walleye 101 photo crew, a commemorative photo album with a group picture and an on the water photo of you and your group (if conditions allow).

Due to the need for early booking of hotels and other items, spots will only be reserved with a $50 deposit per angler per trip. Confirmation will be on a first come, first served basis with deposit receipt date the final determining factor for guaranteed attendance. There will be NO REFUNDS of deposits after the due date for each trip. Any balances due that are not paid by the due date will be subject to a $25 late fee per angler.

Big Fish Pot money will be awarded on Day 1 and 2 only. Each winner will receive half of the total purse. In case of a tie, the day’s money will be split equally among that day’s winners. If there is no fishing on any day due to weather cancellation, the days big fish pot will be raffled at the day’s seminars and anglers must be present to win. Weather cancellations of a day’s fishing can be made by the issuance of an advisory or warning by the US Coast Guard or by the decision of the Walleye 101 staff. A cancelled day will not be counted toward the big fish pot and the above listed rules will apply.

Shirt sizes will be final on payment due date of trip. If you need to change a size after this date, there will be a $20 charge. Changes can be made up to 15 days before the first day of the trip you are attending.

This is an educational event, and due to weather and/or water conditions no fishing time is guaranteed. If no fishing is conducted due to weather and/or water conditions, each day will be filled with learning opportunities including but not limited to, seminars, hands-on clinics, vendor presentations and other educational opportunities. Trips will NOT be cancelled due to weather and/or water conditions, and no refunds or credits will be issued. .

By signing this form you agree to hold harmless “Lance Valentine’s Walleye 101 Inc.” and any and all promoters, agents, agencies, employees and organizations assisting or providing services for the operation of the trip. Furthermore, you agree to waive all responsibility for death, injuries, damages, liability, theft, fire or loss to person and/or equipment of any kind. No unsigned entry will be accepted. A separate entry form MUST be filed for each angler at each trip-no joint entries will be accepted.
